Social Work

Our Social Work Services Department is committed to promoting the total health and well-being of our patients. The department functions as a bridge between physicians, hospital services, the patient and their family.

Social Services provides assistance in many areas including:

• Helping the patient and their family understand the diagnosed illness and issues related to the illness
• Providing physicians with a better understanding of the patient's social and economic environment and how it impacts the patient's medical care
• The social worker is an integral part of the Palliative Care team which emphasizes the patient’s and the family’s rights to participate in an informed discussion and to make choices related to their wishes, needs and plan of care. The social worker is knowledgeable in advanced directives and is available to discuss and complete health care proxy paperwork

Outpatients may contact the Social Work Services Department directly or through their physician. Inpatients may request a referral from their case manager, nurse, or physician.
